HIPAA Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. Substance use disorder treatment records receive additional protection under federal law (42 CFR Part 2).
Our commitment to your privacy
Archangel Centers is required by law to protect the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your health information
- Treatment. To provide and coordinate your care, including with other providers involved in your treatment.
- Payment. To verify benefits, obtain authorization, and bill for the services you receive.
- Health care operations. For quality improvement, training, and other operations that support care.
- As required by law. When federal or state law requires the use or disclosure.
Many uses and disclosures of substance use disorder records require your written consent. We will obtain your authorization before using or disclosing your information for purposes not described in this notice, and you may revoke that authorization in writing at any time.
Your rights regarding your health information
- The right to inspect and request a copy of your records.
- The right to request a correction to your records.
- The right to request restrictions on certain uses and disclosures.
- The right to request confidential communications by a specific means or at a specific location.
- The right to an accounting of certain disclosures of your information.
- The right to a paper copy of this notice on request.
- The right to be notified following a breach of your unsecured PHI.
Our duties
We are required to maintain the privacy of your PHI and to abide by the terms of the notice currently in effect. We reserve the right to change this notice and to make the revised notice effective for the information we already have, as well as information we receive in the future.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
